What Is a Continuum of Care (CoC)?
A Continuum of Care, often called a CoC, is a local planning body that works to end homelessness in a specific region. According to the U.S. Department of Housing and Urban Development, CoCs bring together nonprofit organizations, government agencies, and community partners to coordinate housing and services for people experiencing homelessness or at risk of becoming homeless.
CoCs are responsible for identifying local needs, applying for federal funding, and organizing programs that provide emergency shelter, transitional housing, permanent supportive housing, and other critical services. They also collect data to better understand homelessness in their communities and improve long-term outcomes.
The goal of a CoC is simple but powerful: Ensure individuals and families have access to safe, affordable, and stable housing while addressing the root causes of homelessness.
Who Do CoCs Help?
Continuums of Care serve individuals and families who are experiencing homelessness, including veterans, youth, survivors of domestic violence, and those facing economic hardship. They also support people who are at risk of losing housing by connecting them to prevention resources, rental assistance, and supportive services.
By coordinating efforts across agencies and providers, CoCs create a system where people can move from crisis to stability with fewer barriers.

Executive Director Lakesha Hancock serves on the Board of Directors for Out-Wayne County Continuum of Care, an organization dedicated to ending homelessness in the Out Wayne County area. She also serves on the Governance Committee for H.A.N.D. CoC. These leadership roles strengthen collaboration between Foundation For Pops and regional housing networks, helping ensure that the individuals we serve have access to coordinated housing support and community resources.
